Application for Membership in the
United Pentecostal Church of Pittsburg, California
I desire to become a member of the United Pentecostal Church of Pittsburg, California, having repented of my sins, been baptized in water in the Name of the Lord Jesus Christ (if possible give date . . . . . . . . .) and have received the Baptism of the Holy Spirit with initial evidence of speaking with other tongues as the Spirit gave the utterance (date . . . . . . . . .), or am actively seeking for the Baptism of the Holy Spirit; and that I am a believer in the Apostolic Doctrine as set forth in the Word of God and outlined in the articles of Faith; and, further, I agree to abide by the following rules and teachings of the Bible, which this church stands for, by God's help:
To conduct myself everywhere as becometh a Christian.
For the sake of sinners, weak Christians, yea, and my own soul, I will be an example, a pattern of good works, by abstaining from wordly amusements, (all amusements and recreation are not harmful), watching Television Programs, unclean conversations, intoxicating drinks, or tobacco in any form. II Cor. 7:1; I Thess. 5:22; Rom. 14:21; Matt. 5:16.
To dress discretely, modestly and as becometh holiness at all times, ". . . whose adorning let it not be that outward adorning, but let it be that of the heart, even the ornament of a meek and quiet spirit. . ." I Peter 3:3–4.
Whereas, a woman's long hair is her glory and is given her for a covering, she should allow it to grow without cutting it." I Cor. 11:1–16.
In all other matters pertaining to dress the pastor's decision shall be accepted by all members.
"Not forsaking the assembling ourselves together, as the manner of some is; but exhorting one another: and so much the more, as ye see the day approaching." Heb. 10:25.
To maintain secret devotion, and family devotion whenever permissible in the home.
To support the church and pastor with my prayers and financial aid as God prospers, with the scriptural system of faithfully tithing, and offerings. Mal. 3:10; I Cor. 16:2.
To seek to win the lost to Christ and to manifest a spirit of brotherly love and fellowship to all God's people.
To respect and obey the pastor that he may give account with joy and not with grief. Heb. 13:17.
To abide by the by–laws of the assembly. Tit. 2:1–8.
Where there is no talebearer, the strife ceaseth. Proverb 26:20–22.
